Order Kits for your Office or Drop Ship Kits to your Patients

1. Fill out the form in the button below. You can specify if you want the kits shipped to your office or if you need the kits shipped to your patients. Please fill out the form for each patient you are ordering a kit for. We send collection kits to any location within the continental US free of charge.*
2. For drop ship, our billing team will contact the patient to collect payment information within one business day.
3. Test kits are shipped after payment is collected. Patients can expect their kit to arrive 4-5 business days after collection.

*Please be aware we do not accept samples collected in the state of New York as per regulations.
